City and state at both ends. If there’s a live load or drop, tell us.
Approximate total weight and the biggest piece. If it’s over-dim, say so — we’ll pull permits.
When it’s available and when it has to be there. We’ll come back with a rate the same day.
Pickup, delivery, weight, dimensions — that’s the whole quote form.
